📄 Abstract

The rising prevalence of metabolic disorders necessitates integrative strategies that address the root cause of metabolic sluggishness. In Ayurveda, Hypothyroidism and Obesity are understood through the lens of Sthoulya (Obesity) and Agni-mandya (impaired metabolic fire). This case report describes a 22-year-old male who presented with a 10-month history of significant weight gain, generalized weakness, excessive sweating, and hair fall. Initial biochemical investigations revealed severe Primary Hypothyroidism (TSH: 51.89 µIU/ml) and a pre-diabetic state (HbA1c: 6.3%). The patient was managed with a structured Ayurvedic protocol involving Deepana-Pachana (digestive stimulants), Snehapana (internal oleation) with Panchatiktaka Guggulu Ghrita, and Vamana Karma (therapeutic emesis), followed by Sarvanga Udwartana (dry powder massage). Following the Shodhana (purificatory) intervention, the patient’s TSH levels decreased significantly to 7.45 µIU/ml within three months without conventional thyroxine supplementation. A long-term follow-up at 15 months demonstrated sustained metabolic improvement, with HbA1c normalizing to 5.7% and a stabilization of thyroid function. The patient also reported a significant reduction in subjective symptoms of fatigue and heaviness. This case suggests that Ayurvedic purificatory procedures, particularly Vamana Karma, can effectively modulate endocrine dysfunction and improve insulin sensitivity by clearing Srotorodha (channel obstruction) and restoring Dhatvagni (tissue metabolism). This study highlights the potential of integrative Ayurvedic approaches in managing complex metabolic and endocrine conditions.
