📄 Abstract

Tamaka Shwasa is classically understood as an Amashaya Samudbhava, Kapha-Vata Pradhana and Pranavaha Sroto Dushti Vikara. By standard Shodhana principles, Vamana is the primary procedure for expelling the Dushta Kapha. However, Acharya Charaka presents a classical statement in the 17th chapter of Chikitsa Sthana: “Tamake tu Virechanam”, mandating Virechana prepared with Vata-Shleshma-Hara drugs. This creates a conceptual and clinical paradox about of prescribing Virechana, the principal therapy for Pitta Dosha, for a condition driven by Kapha and Vata. To resolve this paradox, a systematic review was conducted across classical Ayurvedic literature, authoritative commentaries, and 15 peer-reviewed research publications (comprising 9 conceptual reviews, 3 clinical trials, 2 critical reviews, and 1 case report). The findings reveal that the term Pitta Sthana Samudbhava specifically indicates the Adho-Amashaya where impaired Jatharagni produces Ama that circulate to fuel bronchial pathology across the gut-lung axis. Some clinical studies corroborate that Virechana significantly improves subjective respiratory parameters and objective pulmonary function tests while reducing systemic eosinophilic inflammation. Textual analysis shows that Virechana in Tamaka Shwasa is an Avasthiki Chikitsa. While Mridu Vamana is indicated during the acute stage in physically Balavan Rogi with massive Kapha congestion, Virechana is the treatment of choice in Durbala Rogi presenting in the state of Manda Kapha, and in conditions complicated by Udavarta and Adhmana. By establishing Vatanulomana and clearing the Adho-Amashaya, Virechana treats the root of the disease without straining a compromised cardiopulmonary system.
